Πιθανοί κίνδυνοι που κρύβονται πίσω από μία δίαιτα ελεύθερη γλουτένης

Τα τελευταία χρόνια η λέξη γλουτένη, μία λέξη που μέχρι πρότινος πιθανότατα δεν είχαμε ξανακούσει, έχει μπει πλέον για τα καλά στη ζωή μας. Τη βλέπουμε να αναγράφεται στις συσκευασίες των αγαπημένων μας προϊόντων στο σούπερ μάρκετ, τα οποία φέρουν πλέον τη σήμανση «ελεύθερο γλουτένης». Τη βλέπουμε στο μενού του εστιατορίου σε μία ειδική κατηγορία γευμάτων «ελεύθερα γλουτένης». Τη διαβάζουμε στα περιοδικά καθώς η αγαπημένη μας ηθοποιός δήλωσε πρόσφατα ότι ακολουθεί μία δίαιτα «ελεύθερη γλουτένης» στην οποία μάλιστα και οφείλεται η αψεγάδιαστη σιλουέτα της και η άριστη κατάσταση της υγείας της. Και φυσικά την ακούμε να την συζητάει η παρέα δίπλα μας στην καφετέρια λέγοντας χαρακτηριστικά ότι «έφαγα μία ολόκληρη συσκευασία μπισκότα σήμερα για σνακ, αλλά δεν πειράζει γιατί ήταν ελεύθερα γλουτένης». Τι γίνεται τελοσπάντων με αυτή τη γλουτένη. Βρήκαμε επιτέλους την πηγή του κακού; Βρήκαμε επιτέλους την ουσία αυτή που μας προκαλούσε εδώ και χρόνια ενοχλητικά φουσκώματα, έκανε τη ζυγαριά να ανεβαίνει χρόνο με το χρόνο και απειλούσε σιωπηλά την υγεία μας χωρίς εμείς να το γνωρίζουμε; Ή μήπως όλο αυτό είναι μία μεγάλη παρεξήγηση η οποία αποδίδεται στην  έμφητη ανάγκη μας να βρούμε τον αποδιοπομπαίο τράγο στον οποίο να φορτώσουμε όλα τα προβλήματα σχετικά με τη διατροφή και την υγεία μας τα οποία δεν μπορούμε μόνοι μας να διαχειριστούμε;

Ας τα βάλουμε επομένως τα πράγματα σε μία σειρά και ας προσπαθήσουμε να διαχωρίσουμε μέχρι που φτάνει η αλήθεια και τα δεδομένα και που ξεκινάει ο μύθος και η παραπληροφόρηση. Η λέξη γλουτένη είναι πλέον γνωστή σε όλους μας, λίγοι όμως είναι εκείνοι που γνωρίζουν τη σημασία της. Η γλουτένη είναι μια πρωτεΐνη που βρίσκεται στο σιτάρι, το κριθάρι, τη σίκαλη και στο τριτικάλε (συνδυασμός σιταριού και σίκαλης) η οποία αποτελείται από τη γλοιαδίνη και τη γλουτελίνη.  Λειτουργεί ως «κόλλα» σε τρόφιμα όπως τα δημητριακά, το ψωμί και τα ζυμαρικά, βοηθώντας τα να διατηρούν το σχήμα τους. Η γλουτένη μπορεί επίσης να βρεθεί σε ορισμένα καλλυντικά προϊόντα. Ο οργανισμός ορισμένων ανθρώπων μπορεί να αντιδράσει στη κατανάλωση της γλουτένης, μία κατάσταση η οποία ονομάζεται δυσανεξία στη γλουτένη ή αλλιώς κοιλιοκάκη. Στην κοιλιοκάκη, η γλουτένη προκαλεί μια ανοσολογική αντίδραση που προσβάλλει το επιθήλιο του λεπτού εντέρου. Αυτό σημαίνει ότι ο οργανισμός δεν είναι σε θέση να απορροφήσει αποτελεσματικά τα θρεπτικά συστατικά στην κυκλοφορία του αίματος και μπορεί να οδηγήσει σε αναιμία, καθυστέρηση της ανάπτυξης και απώλεια βάρους. Συμπτώματα της κοιλιοκάκης είναι φλεγμονή στο έντερο με διάρροιες, κοιλιακό πόνο, κράμπες και τυμπανισμό. Σε αυτά τα άτομα αν αφαιρέσουμε εντελώς την γλουτένη από την διατροφή τους δεν αντιμετωπίζουν πρόβλημα και μπορούν να ζήσουν φυσιολογικά. Η συχνότητα της κοιλιοκάκης στην Ελλάδα πιθανολογείται ότι είναι 1/2.000-3.000 κατοίκους.

Αν κοιτάξουμε τις κλινικές μελέτες θα δούμε ότι υπάρχουν ορισμένες ενδείξεις ότι η γλουτένη μπορεί να δημιουργεί προβλήματα ακόμα και σε άτομα τα οποία δεν πάσχουν από κοιλιοκάκη. Συγκεκριμένα η μελέτη του Αυστραλιανού Peter Gibson το 2011 έδειξε ότι η δυσανεξία στη γλουτένη που δεν σχετίζεται με κοιλιοκάκη, μπορεί να αποτελεί πραγματική διαταραχή, καθώς  διαπίστωσε ότι συμμετέχοντες στην έρευνα που κατανάλωναν γλουτένη παρουσίαζαν φουσκώματα και κόπωση. Δύο χρόνια αργότερα όμως μία νεότερη, καλύτερα σχεδιασμένη, μελέτη από τη ίδια ομάδα ήρθε να ανατρέψει τα προηγούμενα αποτελέσματα καταλήγοντας στο συμπέρασμα ότι δεν υπάρχει κανένα στοιχείο ειδικών ή δοσοεξαρτώμενων επιπτώσεων της γλουτένης σε ασθενείς με δυσανεξία στη γλουτένη που δεν σχετίζεται με κοιλιοκάκη. Επιπλέον, οι ερευνητές απόδωσαν τα συμπτώματα των φουσκωμάτων που είχαν προηγουμένως παρατηρηθεί στην κατανάλωση μη απορροφήσιμων – ζυμώσιμων υδατανθράκων των επονομαζόμενων FODMAPs.

Αξίζει επομένως τον κόπο για μερικές ενδείξεις να αποκλείσουμε μία ολόκληρη κατηγορία τροφίμων τα οποία περιέχουν γλουτένη από τη διατροφή μας ή μήπως ο περιορισμός αυτός μπορεί μακροπρόθεσμα να φέρει περισσότερα αρνητικά παρά θετικά αποτελέσματα. Τα επεξεργασμένα τρόφιμα χωρίς γλουτένη, εξαιτίας της μεταποίησής τους, στερούν από τον καταναλωτή τα πολυάριθμα οφέλη για την υγεία των δημητριακών ολικής άλεσης και μπορεί μακροπρόθεσμα να προκαλέσουν σοβαρές ελλείψεις σε πολύτιμα θρεπτικά συστατικά όπως φυτικές ίνες, σίδηρο, ψευδάργυρο, φυλλικό οξύ, νιασίνη, θειαμίνη, ριβοφλαβίνη, ασβέστιο, βιταμίνη Β12 και φώσφορο. Τα οφέλη της κατανάλωσης δημητριακών ολικής άλεσης έχουν επιβεβαιωθεί εδώ και δεκαετίες μέσα από μια σειρά μελετών οι οποίες δείχνουν ότι τα άτομα που καταναλώνουν προϊόντα ολικής άλεσης είναι λιγότερο πιθανό να είναι υπέρβαρα ή να πάσχουν από ασθένειες όπως ο διαβήτης, οι καρδιακές παθήσεις ακόμα και ο καρκίνος. Επίσης τα δημητριακά ολικής άλεσης περιέχουν 31 διαφορετικά αντιοξειδωτικά τα οποία προστατεύουν τον οργανισμό από τη νόσο και τη διαδικασία της γήρανσης που προκαλούνται από την οξείδωση. Από την άλλη, μία διατροφή η οποία είναι εκ φύσεως χωρίς γλουτένη ή χαμηλής περιεκτικότητας σε γλουτένη μπορεί να είναι παράλληλα και υγιεινή εάν είναι σωστά σχεδιασμένη. Ένα καλό παράδειγμα είναι το μεγαλύτερο μέρος της Ασίας, όπου η βασική τους τροφή είναι το ρύζι και όχι το σιτάρι. Ας μην ξεχνάμε όμως ότι και το ρύζι και οι πατάτες οι τηγανιτές είναι εκ φύσεως ελεύθερα γλουτένης χωρίς αυτό να σημαίνει ότι μπορούν και τα δύο να αντικαταστήσουν το σιτάρι στα πλαίσια μίας υγιεινής διατροφής.

Η διατροφή που θα ακολουθήσει ο καθένας μας είναι ένα ζήτημα καθαρά προσωπικό. Προτού όμως αποφασίσετε να αλλάξετε τον τρόπο διατροφής σας υιοθετώντας ένα ειδικό σχήμα διατροφής, όπως η διατροφή ελεύθερη γλουτένης, ενημερωθείτε σωστά και από έγκυρες πηγές σχετικά με τις αλλαγές που θέλετε να εφαρμόσετε. Ο γιατρός σας είναι ο υπεύθυνος να αξιολογήσει τυχόν συμπτώματα του γαστρεντερικού σας συστήματος ενώ ο διατροφολόγος σας μπορεί να σας προσφέρει έγκυρη ενημέρωση και σωστή καθοδήγηση προς την ανακούφιση των συμπτωμάτων αυτών αλλά και προς μια υγιεινή και ισορροπημένη διατροφή βασισμένη στα θέλω σας και τις ανάγκες σας.
